Direct Specialty Care Alliance

Our Mission & Values

Why we exist, what we stand for, and what we will never compromise. The DSC Alliance was founded on one conviction: medicine works best when the physician's only obligation is to the patient. Not to an insurer. Not to a hospital system. Not to a billing quota.

"To restore physician autonomy and rebuild the doctor-patient relationship — by building the community, education, and platform that empowers specialist physicians to practice on their own terms, and patients to find them. Because Change Starts With Us."

Two sides. One mission. For physicians
Everything needed to leave the insurance system and build a sustainable, autonomous practice — education, mentorship, community, and a national directory listing. For patients: a growing directory of independent specialists who have time for them, are transparent about fees, and answer to them — not an insurer. As the physician community grows, so does the patient directory. The two are inseparable.
Medicine used to be simple:
A physician applied their knowledge, a patient received care, and that relationship was everything. Then a third party inserted itself — claiming authority over clinical decisions and turning a healing profession into a billing operation. The DSC Alliance exists to undo that.

Where the Mission Came From

The mission did not begin with a business plan. It began with a patient — a 42-year-old teacher who spent over a year being told his pain was psychological. When an Alliance co-founder finally had the time to sit with him and take a full history, the diagnosis was clear: seronegative rheumatoid arthritis. He had been suffering unnecessarily because the system was designed to bill, not to heal.

His case was not exceptional. It was the norm. Specialists seeing 30 patients a day. Appointments running 10 minutes. Physicians who trained to heal — buried in documentation and insurance battles.

In 2019 one of our co-founders left insurance-based medicine and launched one of the first Direct Specialty Care practices in the United States. More time. Better outcomes. Patients who felt heard. Medicine that felt like medicine again. In 2020 the DSC Alliance was founded — not as a business venture, but as the community she wished had existed when she made the leap.

Our Core Values

These are not aspirational statements. They are the principles that define what it means to practice Direct Specialty Care.
01
Patient First

The patient is not a billing unit or a slot in a 30-patient schedule. Every DSC physician has chosen to put patient well-being ahead of insurance convenience, hospital administration, and RVU targets. This is the reason they became doctors.

02
Transparency
Every DSC physician publishes their fees before the first appointment. No estimates. No surprise bills. What the physician says it costs is what it costs. Transparency is not a feature of DSC — it is a non-negotiable standard.
03
Accountability
The Direct Care Contract concentrates accountability where it belongs — between the physician and the patient. No insurance company to hide behind. No hospital system to absorb responsibility. What the physician commits to, they are legally required to provide.
04
Autonomy
Physician autonomy is a patient safety issue. When clinical judgment is overridden by an insurance policy, patients receive the care the insurer decided to pay for — not the care the physician determined they need. DSC physicians follow their medical training. Not a coverage manual.
05
Accessibility
High-quality specialist care should not be a luxury. DSC fees are set at fair market value — not inflated by insurance overhead. Accessible pricing is not charity. It is a professional standard every Alliance physician commits to.

What We Believe

"Medicine used to be simple. A physician. A patient. A direct relationship. Something broke that. We are here to fix it. Because Change Starts With Us."

The system’s failure to honour why physicians became doctors is not a personal failing — it is a structural one that demands a structural solution.
Patients have a fundamental right to know what their care costs before they receive it.

A profession that cannot defend its autonomy cannot protect its patients.
Direct care is not a fringe alternative. It is the future of medicine — and it is already here.

The physician-patient relationship is the most powerful therapeutic tool in medicine. Every barrier placed between them is a harm.
The DSC movement grows stronger with every physician who joins, every patient who finds care, and every story of what medicine can be when nothing stands in the way.

Our Standards for DSC Alliance Members

The DSC Alliance is a physician-only organisation.

Membership requires genuine alignment with our mission — not just agreement with it. Every DSC Alliance physician commits to:

  • Providing excellent care with transparent, affordable pricing at fair market value
  • Offering a Direct Care Contract that defines fees, services, and access terms upfront
  • Prioritising direct physician-patient communication and continuity of care
  • Designing their practice around patient needs — not recurring revenue
  • Helping patients understand all options for labs, imaging, and medications — insurance and direct-pay
  • Contributing knowledge, experience, and support to the Alliance community
“True medicine is about caring, not just counting. By embracing new ways of practicing medicine, we can return to the heart of our medical mission — where our knowledge and dedication are valued and recognised for their difference in people's lives.”
— Dr. Diana Girnita, Co-Founder, DSC Alliance

Two practicing specialists left the insurance system, proved direct care worked, and realised every other specialist making the same transition was starting from zero — no community, no resources, no roadmap. The Alliance exists so no specialist ever has to start from zero again.
